First Visit Guide
Please arrive 10-15 minutes early for your appointment for registration and to fill in a health questionnaire.
When you come for your appointment, please bring the following:
- Driver’s License or a valid ID
- Insurance information
- Referral Letter (if required)
- Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
- List of medications (if any)
If you need to cancel an appointment
Telephone the office during business hours and allow at least 1 days' notice so that we can offer your appointment time to patients on our waiting list.
We recognize that your time is valuable, and we make every effort to run on time. Occasionally, emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.
Parking and Entry Information for OrthoEnglewood
- Garage Parking: The attached garage entrance is on S. Dean Street before the building and also on Englewood Avenue. Please park on "Level I" (next level up from ground level) of the garage for easy access to "Level I" entrance. Garage parking requires payment via the Park Mobile App (Zone #9649) or a payment machine located at the ground level by the elevator and by the garage door entrance for the building. The cost is $.50 for the first hour and $1 each hour after.
- Street Parking: Available on S. Dean Street or Englewood Avenue. Enter at the front of the building.
- Entering the Building: On the monitor to the side of the garage and/or front door, press contacts and press the green call button for "OrthoEnglewood." You will be buzzed in. If entering through the front door, please take elevators at the end of the hall through the glass doors to the 2nd floor. Our office is to the left down the hallway.
